They would also like to engage a Registered Nurse on a part time basis who will possess specialized training in the area of mental health and addictions. Description: The purpose of this Project is to allow an Elder to accompany Na’Me’Res’ Outreach and Aftercare staff in their activities.  The Outreach and Aftercare workers travel the city in our vans offering the following services:

(i)                 Outreach – The aboriginal specific outreach and support services provides a confidential case management service which addresses the basic needs of Aboriginal persons who are homeless, living on the streets or in seasonal shelters.  We provide traditional and culturally specific supports which assist clietns to affect a life away from homelessness, toward a self-sufficient lifestyle.

(ii)               Aftercare – is for the Aboriginal clients who have made the transition from the streets to living safely and independently with a connection to traditionally specific supports.  Aftercare assists in helping clients maintain a life away from the streets by assisting them to integrate into their neighbourhood and access various supports and resources.  This will be accomplished through general client support, housing maintenance, eviction prevention, facilitating landlord-tenant relationships, linking to physical and mental health care and facilitating access to other services.

They will also liaise with Aboriginal and Non-Aboriginal agencies that will meet the cultural and life needs of clients.

Description: Capital Renovations to the property at 22 Vaughan Road is owned by Na-Me-Res. It is being leased to Anishnawbe Health Toronto who will deliver the ‘Toronto Aboriginal Concurrent Disorders Program’. Clients of their Shelters and After Care Centre will greatly benefit from the services which will be offered. Previous renovations did not include the garage or breezeway. Their request is for funding to demolish the existing breezeway and replace it with a 6” privacy fence. This will provide privacy to the area for ceremonies. The second item is a new air conditioning unit. The unit will ensures a steady flow of fresh air, minimizing the risk of communicable diseases including TB which has impacted on the Aboriginal community. The final request is for lockers for the residents to store their personal belongings. Theft is a common occurrence in shelters and this would help prevent theft of items such as ID.

Description: Native Men’s Residence is looking to hire a consultant to design and develop a culture based service delivery model which meets the needs of their clients.  At the end of this project, it is expected that Native Men’s Residence will have the capacity to provide special needs clients with the care and support they require. This will also provide a long term sustainability plan for the organization.

Description: The goal of this project is to assess the possibility of developing a withdrawal management service that relates specifically to the needs of urban Aboriginals with substance abuse problems and to design such a service so that it will have a high probability of success in providing the pathway to sobriety and success for Aboriginals in Toronto for whom substance abuse has been a barrier to success.

Description: Develop a comprehensive strategic plan which will serve the organization over the next three to five years to better combat homelessness.  The strategic plan will be based on community needs when it comes to fighting homelessness and changes in the environment (agencies and funders) and enhanced capacity at Na-Me-Res.

Description: This is a capital project to provide renovations to Na’ Me’ Res’.  Replacement of broken doors, and replacement of concrete along north wall which is causing leaking in the building ~ waterproof the basement.

The overall goal of these two components is to provide a benefit to the environment, reduce operating costs, and improve safety and security.
